Extra pitch added when pitchers change mid-at bat

I've had a number of situations where iscore seems to tack on an extra pitch to the departing pitcher's count when they leave in the middle of an at bat.
I've tried to replicate at home with a test game, but I don't seem to be able to. However, the situation is relatively common during regular games, so I've seen it a half-dozen times.
Is this a known issue? Any advice on how to best test (case management tool etc?)
My son's league is super-freaky about pitch count. So much so that when a player reaches his limit he has to stop, even if it is full count. If I turn in numbers saying that a kid threw 76 instead of 75 pitches it can have huge implications. It sounds trivial but some overly competitive parents take it *very* seriously.
