Courtesy Runner Question

Pitcher reaches first base safely, then gets subbed out for a courtesy runner. That courtesy runner then scores. Who gets credit for the run?

What are the general guidelines to determining who gets the error when the throw to first isn't great? What if the throw to first is on line but low? It doesn't bounce before getting to first base but it's right at the shoe tops and then not caught. Is that a throwing error on the fielder or an E3? ...

How do I interpret the error codes printed on the scorecard? For example, during a recent game we had runners at 2B and 3B. Batter hit a fly ball that was dropped by the CF for an error.
