It happens very often when the players come up for the third time. You can temporarily fix it it by tapping the plate appearance information area a couple times, though. This is pretty annoying, but now at least i know that it is flakey.

When I view the pdf version of the scorecard, the total pitches thrown by the pitchers always reads zero. Is anyone else having this problem?
